Enrollment process for students NEW to SMUSD

Enter your address in our SchoolSite Locator to confirm that your address falls within the attendance boundaries of San Elijo Middle School.


Click here to enroll a student NEW to SMUSD for the 2021-22 school-year

  • Read "2021-22 Annual Parent Notifications"  here
  • Read "Home Language Survey" here

Gather & Attach these documents to your enrollment packet:

  • Birth Certificate (must be Original with State Seal to be accepted as proof of age) or passport
  • Immunization Record (Yellow Immunization Card)
  • Two [2] Proofs of Residency (one [1] from each category, dated within the last 60 days)
    • Mortgage statement, lease agreement, or property tax bill
    • Utility bill, government correspondence, or pay stub 
  • Check-out Form from previous school
  • Last recorded grades and/or state test scores
  • If applicable, IEP or 504 Plan (We must have this document before we can enroll your student)

Enrollment is processed AFTER ALL required documents have been submitted
SEMS has a 48-hour Enrollment Processing Window
